PLAE Exemplar Awards Call For Nominations

Good day

The year 2016 marks  another year  to  celebrate  the  Epilepsy Exemplar Awards  as the  Philippine  League  Against  Epilepsy  (PLAE)  recognizes  our  patients  with epilepsy. PLAE  aims to  honor  Persons With  Epilepsy  (PWE) who  have  achieved excellence in their chosen fields by rising above their adversities.

PLAE  is  partnering  with HI-EISAI Pharmaceuticals  in  this  event.  Through  this partnership, we will be able to provide great opportunities for our PWEs in helping them achieve their dreams.  Similar to the most recent Epilepsy Exemplar Awards, each  awardee  will  be  receiving  a  prize  tailored fit  to  support  his/her  personal development, which will be in the form of an educational grant.

To  this  end,  we  are  inviting  all  PLAE  members  to  nominate  patients  who  they believe best represent what the Exemplar Award stands for.  The nomination forms and the criteria for nomination are enclosed in this letter.  In order for us to have enough time to review all nominees and finally select the winners, we would like to request that you send in your nominations before June 30, 2016.

EXEMPLAR AWARDS 2016 – CRITERIA FOR JUDGING

1. The contest is open to all Filipino citizens between the ages of 8 and 65 years old who have been diagnosed with epilepsy.
2. He/She must possess a pleasing personality and good oral communication skills.
3. He/She must be willing to act as spokesperson and role model for persons with epilepsy.
4. He/she must have good seizure control and ideal patient behaviour in complying with his/her medical treatments.
5. He/She must have a positive attitude toward his/her condition such as optimism, self-determination and independence that allows him/her to live an independent life.
6. He/She must have academic/professional achievements.
7. He/She must be endorsed by his/her Neurologist.

Patients who wish to enter this search may be nominated in writing by his/her family, friends, teachers, classmates or co-workers.

The Philippine League Against Epilepsy Inc. Is a non-profit national organization of professionals committed to the improvement of the quality of life of persons with epilepsy through education, research, prevention, advocacy and delivery of optimal care. The “Epilepsy Exemplar Award” is part of the continuing national epilepsy campaign “Epilepsy: Out of the Shadows PHILIPPINES”.
